1. Why The Plan Has Been Made

Valuing People is a plan from the Government for improving the lives of all people with learning disabilities.

In Herefordshire a plan was needed to make sure that things get better here.

Valuing People is based on 4 main beliefs :-

Having Legal and Civil Rights

People with learning disabilities have the same rights as other people

Independence

People with learning disabilities should be given the support they need to be as independent as they want

Choice

People with learning disabilities can speak up from themselves and make their own choices about where and how they live

Being Included

People with learning disabilities should be able to use and be included in all the services in their own community

 

2. Herefordshire Learning Disability Partnership Board

Valuing People say that everyone should work together to make sure that people with learning disabilities can have better lives.

To do this in Herefordshire there is a group of people who meet to look at better ways to plan services. There is a list of these people at the end.

There is a lot of work to do so the Partnership Board has set up smaller groups to look at different things. These things are:-


In 2005 the Partnership Board asked these groups to think of 3 or 4 targets that they would make sure happened by the middle of 2006. They called this work –

The Herefordshire Learning Disabilities Partnership Board Plan 2005.

3. Plan Number 2 - July 2006

The Partnership Board then asked these groups to think of some new targets that they would make sure happened by the end of 2006. The next pages show what the groups wrote.

 

2006 Targets

Here are the 3 or 4 important things each group has said will happen before the end of December 2006

4 Better planning for young people leaving school

1.

For the new Transitions Lead and Transitions Worker in Connexions to start work in September 2006.

2.

For the organizations involved in Transitions to agree a plan about what the Lead and Worker will do and by when.

5 More Choice and Control

 

1.

Write a leaflet on how to look after money.

2.

Set up a course with Lifelong Learning on how to look after money.

3.

The People’s Union and local Mencap will tell everyone about National Mencap’s campaign “Make it Clear” .

4.

Make sure the Bill of Rights is promoted throughout the county by training a group of people from the People’s Union to make sure the Bill is making a difference by:

    • Training staff, carers and other people who work with people with learning disabilities about how the Bill should work

    • tell everyone with a learning disability about the Bill

    • monitor how the Bill is working

6 Person Centred Planning

1.

leafletA Person Centred Planning Leaflet is available for distribution at key sites and at information days

2.

Develop an eligibility criteria for Person centred planning

3.

Develop a strategy for delivering PCP facilitation training so that the maximum number of services/ organizations etc. will nominate staff to attend.

7 Supporting Carers


To find out if carers can have a health check each year. (Carers’ UK found that most carers want this).

Checks would include:

  • Blood pressure

  • Diabetes

  • Mental health – stress/worry levels of carers

  • Flu jab

  • Signposting for such things as benefits advice, general support etc.

The Carers’ Network will choose 5 GP practices and find out if health checks are available at any of the surgeries and if carers would attend events such as ‘well carer’ clinics or special ‘healthy carer’ days.

8 Health

The group want to finish off last year’s targets, which were:

  • To make sure that all people who have a learning disability have a GP and that GPs know all the people who have a learning disability in Herefordshire.

  • People who have high support needs should all be offered a health assessment and a Health Action Plan within 1 year.

All health information leaflets should be available in easier-to-understand leaflets.

9 Housing

For the Housing Group to start again and be reference group for The Accommodation and Support Manager in the Learning Disabilities Service (Lydia Bailey).

10 Fulfilling Lives

1.

To support Laura in her role as Day Opportunity Development Manager and to comment on proposals either in support or against the recommendations.

2.

To communicate information relating to the Modernisation of Day Opportunities to stakeholders.

11 Work

1.

To develop a system to collect data on people who are eligible for a Council funded learning disabilities service, who are in paid or voluntary employment.

2.

To increase the number of people with learning disabilities who are known to the Council who are in paid work of 5 hours or more per week by 10%.

3.

To increase the number of people with learning disabilities who are known to the Council who are in voluntary work.

12 Quality

1.

To increase the number of people who use services on the sub-group.

2.

To develop a series of questions relating to service quality and see if we get the same sort of questions as ‘My Life’. These questions have to be about real things – ‘outcomes’ – and not just things written down by someone.

3.

To support & train members of the sub group to use these questions to check quality in 1 or 2 services.

4.

To seek a way of ensuring that these people are paid to do this.

5.

To check back on how things are going and see if we can make the quality of the audit better – and then see if other people want to use it, or something like it.

13 High Support Needs

There will not be a High Support Needs group. All the other groups will think about the needs of people with high support needs when they are making plans.

14 Workforce and Planning

1.

The Partnership Board will set up the Workforce Group.

2.

The group and start by finding out more about the learning disabilities workforce in the Council, PCT, voluntary and independent services.

15 Partnership Working

1.

Make sure all the sub groups are working fully and reporting back to the Board.

2.

Make sure the website team is given all the information it needs.
